Empowering Community Health
Health promotion is the process of empowering people to take control over and improve their well-being. It represents a comprehensive process, which includes support from dedicated Community Health Workers that provide education and resources that address the social determinants of health, including social, environmental, and economic conditions that effect a person's health outcomes.
We are here to assist you in your health journey beyond your medical needs. These services are offered at no cost to you.
Our Community Health Workers can assist with:
- Covered California and Medi-Cal application assistance
- Every Woman Counts program enrollments
- FamilyPACT program
- CalFresh program enrollment assistance
- Oral health education for Adults and Children
- Information and referrals to community resources:
Prenatal care resources
Sexual & reproductive health resources
Resources for climate-sensitive or environmental related health Issues
Resources related to aging
Resources for domestic violence
Resources for housing assistance
Resources for economic hardship
Sliding Fee Scale Program
