Enhanced Care Management
ECM at SBNC
Enhanced Care Management (ECM) is a healthcare benefit through CenCal that provides extra support and coordination for individuals with complex medical and social needs. This includes access to a dedicated care manager who works to address both the clinical and non-clinical challenges, often connecting the patient with community resources to improve your overall health and well-being.
